The Acute Medicine Clinical Social Worker functions as an integral member of an interdisciplinary team to maximize the Veteran's strengths and identify treatment goals. Incumbent provides psychosocial services to the Veteran population admitted to Acute Medical settings. Services include assessments of psychosocial situations/ needs, and coordinating discharge planning for Veterans who may have serious co-morbid medical problems, mental health problems and complex psychosocial issues.
