OverviewSt. Luke's Health System is seeking a Social Worker to work in our on site Virtual Care Center in beautiful Boise, Idaho! This Social Work position is a remote patient management role and is the latest evolution of home based monitoring-connecting patients with a care team tracking warning signs. Patients have a daily connection with nurses and social workers who monitor vitals and look for behavioral signs such as depression or anxiety. This role is integrated into our Virtual Care team and works directly with a team of professionals. The Social Worker will work with patients that have chronic disease.
