The Medical Foster Home Program Coordinator is a social worker who is responsible for maximizing psychosocial quality of life issues in order to assist veterans who need a nursing home level of care have the benefits of a less restrictive level of care such as a Medical Foster Home. In addition, this position is responsible for the administration of the Adult Day Health Care Program and the provision of social work case management services to Veterans.
