Job Summary
The Hospital Recovery Coordinator will function as a liaison between Harris Health Systems and Harris County Public Health-Hospital Violence Interruption Program (HVIP). The Hospital Recovery Coordinator will facilitate the collaborative multidisciplinary process of therapeutic case management encompassing trauma-informed care, assessments, and planning to empower patients hospitalized due to violent intentional injuries. The Hospital Recovery Coordinator will be responsible for the provision of intensive crisis intervention, ongoing advocacy, and case management to victims and survivors of violent intentional injury and their families/support network during hospitalization. The Hospital Recovery Coordinator will identify options and advocate for services to meet the patient's and family's comprehensive needs in collaboration with Violence Interruption staff, in efforts to prevent future hospitalizations. The SWCM I participates in quality improvement activities, exemplifies professionalism, and promotes a customer-friendly environment by utilizing ServiceFIRST behaviors in interactions with patients, families, and staff members.
Minimum Qualifications
Degree:
a) Bachelor's Degree
b) Master's Degree in Social Work
Licensure/Certification:
a. License in Social Work: Licensed Master Social Worker (LMSW) or
Licensed Clinical Social Worker (LCSW)
Work Experience:
a) One (1) Year Work Experience: Experience in working in a health care setting required. Experience working in a violence prevention/intervention program, preferred.
