This social worker is assigned to outpatient geriatric patients with primary responsibility to provide direct services to the Contract Nursing Home Program (CNH), Home based care (HBC), State Veteran Home (SVH) and other Geriatric Care Programs offered to veterans and families who have need for psychosocial intervention. Additional responsibility to provide assistance to VA Contracting Official(s) for the CNH program by serving as the Contracting Officers Representative (COR) for the program. The incumbent functions within a multidisciplinary team comprised of a social worker, registered nurse, and other clinical specialists.
